Mandatory COVID-19 Protocols for Patients
 

Please do not come in if yourself or a family member is sick.

If yourself or a family member is sick, please do not book an appointment until symptoms have resolved for 14 days.

Pre-Screening: A pre-screening agreement will be e-mailed to patients 24hrs prior to their appointment.

Failure to respond to this will result in the cancellation of your appointment.

All clients must wear a mask into the clinic. Please bring one if you can.

In the case that you cannot, a disposable mask will be provided.

All clients entering the clinic must sanitize their hands immediately upon arrival, there will be sanitizer readily available.

 

COVID-19 screening on arrival;

- temperature reading

- signed COVID-19 Screening form stating that you do not have any COVID-19 symptoms.

Contact-less forms of payment (debit, credit, e-transfer)
